No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. All the above categories exclude text messaging originator opt in data and consent. This information will not be shared with any third parties.
Opt In Information
By providing your phone number, you consent to receive text messages from us. You can opt out at any time by replying STOP.
Opt Out Instructions
You can cancel the SMS service at any time. Text STOP to our number. After you send STOP, we will send an SMS message to confirm that you have been unsubscribed. After this, you will no longer receive SMS messages from us.

Information You Choose To Provide
KATYCS may receive your name, business contact information, company or practice name, business role, and the answers you provide when requesting a service, assessment, or review. If a program offers text messages and you choose to join, KATYCS may also receive your mobile number and the record of your consent.
Information Collected Through Website Use
The website and its hosting providers may receive standard technical information such as browser type, device type, pages viewed, referring page, and approximate network location. KATYCS may use this information to operate, protect, and improve the website.
How Information Is Used
KATYCS may use submitted information to respond to a request, prepare and discuss an assessment, provide an agreed service, maintain business records, protect the website, and communicate about services when the recipient has provided the required consent.
Growth Review Drafts And Submissions
A Growth Systems Review draft is saved only in the visitor's current browser until the visitor submits it, clears it, or the 30 day draft period ends. A completed request is stored by the website hosting provider so KATYCS can review the answers, prepare the discussion, and maintain the request record. The confirmation includes a reference number and the initial areas KATYCS will examine.
Service Providers And Required Disclosures
KATYCS may use service providers that support website hosting, communications, scheduling, analytics, or business operations. They may receive only the information reasonably needed to provide their service. KATYCS may also disclose information when required by law or when reasonably necessary to protect rights, safety, or the integrity of the service.

Public KATYCS forms are for business level information only. Do not submit patient information, raw schedules, appointment dates, clinical data, insurance or payer information, credentials, or other protected records. Medical Capacity Snapshot information is requested only through the approved secure process.
